Abyss | 《当呼吸化为空气》
作者保罗，游走、徘徊于人文哲学和神经医学两个领域。最终，还是以神经医学，这个控制人思考和行为的医学，支撑作者保罗的人文哲学思考。
一、作者服用Tarceva治疗肺癌
最近上映的电影《我不是药神》，票房很火。电影里提到的抗肿瘤药格列宁最后进了医保，价格由一个月的费用高达2.3万至3.9万；直到2017年才纳入我国医保药品内，价格将到几百元，病人受益。
现实生活中，还有一个抗肺癌，特罗凯（厄洛替尼Tarceva），一个月一片价格是1.8元，纳入医保后，个人负担1260元/盒。
回到这本书《when breath becomes air》（当呼吸化成空气），作者经过两次治疗，第一次服用就是特罗凯Tarceva。

该药由罗氏公司Roche研制成功，用于治疗肺癌和胰腺癌。罗氏2017年年度报告也披露，在中国大陆的四个抗癌药，包括Tarceva在内，2017年都进入医保名单National Reimbursement Drug List (NRDL)：
In China, four of our cancer medicines—Herceptin, MabThera/Rituxan, Avastin and Tarceva—were included in theNational Reimbursement Drug List (NRDL) as of 2017. For the first time, cancer patients all over China will have access to these potentially life-saving treatments. This agreement came as a result of Roche’s systematic approach of identifying local access hurdles and creating a tailored local plan.
二、绝望
保罗服用Tarceva治疗失败。继而采用化学疗法Chemotherapy。
Chemotherapy这个单词是由Chemo，chem/o，chemical 化学的，加上therapy ，treatment治疗的意思，Chemo-therapy，就是化学疗法。

最终，化疗效果也不好。书的最后，治疗医师Emma，他在医院的同事，安慰他还可以存活5年（you have five good years left）。
作者保罗仰天而叹，不禁感慨。医生Emma祈求的口吻说自己可以再活5年，也只是一种安慰。他描述当时的状态是：
There we were, doctor and patient, in a relationship that sometimes carries a magisterial air and other times ,like now, was no more, and no less, than two people huddled together ,as one faces the abyss.
当下的我俩，医师与病人，我们的关系有时如管理者与被管理者（气氛比较严肃）；有时则如此刻，不多也不少，只是两个人相互打气，而其中一个人的脚下就是深渊（面临死亡的深渊，abyss）。
三、Abyss，进入深渊
Abyss这个词很有意思。意思是峡谷，a chasm；深到没有底儿，a bottomless depth，字面来看，指地球上的一个大裂痕，it describes a fissure of great depth in the earth’s surface.更常见的是，它表达一个不能测量、不能衡量的深度、程度。

肯尼迪的Absyss
美国总统肯尼迪（Kennedy）在1961年有一个演讲，提到
But however close we sometimes seem to that dark and final abyss, let no man of peace and freedom despair. For he does not stand alone.

他在遇刺身亡两个月前在联大发表了他生平在联合国的最后一次演讲，此次演讲论述了和平这一主题，当时所面临的根本性挑战是如何克服核战争，dark and final abyss， 因为核武器，人类可能陷入无限深渊。
丘吉尔的Abyss
英国首相丘吉尔（Churchill）1940年在下议院（House of Commons）发表演讲，提到
If we fail, then the whole world will sink into the abyss of a new Dark Age, made more sinister, and perhaps more protracted, by the lights of perverted science.

Abysm，很少用的一个形式，名词，深渊无底洞；Absymal，形容词，深不可测。
例子：the abysmal poverty of much of the Third World should shock the conscience of developed peoples. 第三世界极度的贫困应觉醒发达民族。
例子：The abysmal ignorance in which the people are kept is one of the strongest weapons of dictators. 人们极度的无知，是独裁者们的最强有力武器。
最后，
作者保罗的文笔，如他做神经外科肿瘤手术一般，游刃有余，精而雅。他服抗癌药和化疗无望后，仰天而叹，用了一个词Abyss。
保罗文笔精美，医学文学两徘徊。于医学、于英文都是非常好的一本书，值得精读！
